-WE ARE SCENT FREE AT FVNM-Please refrain from wearing perfume, cologne, essential oils and any scented products to include laundry products when visiting the office.
-Prior to EVERY appointment, you must complete your Symptom Review Questionnaire and/or Brief Pain Inventory.
-NEW PATIENTS - complete ALL questionnaires under the "Forms and Questionnaires" section & notify us when completed to schedule your 90 minute New Patient appointment.
-Upload a profile photo in your portal.
-ALL of the questionnaires must be re-submitted ANNUALLY.
